Description

30 bottles with compressed hydrogen fell from a truck at a junction, because of speeding. The bottles started releasing hydrogen. Emergence team was called, and leak was stopped.

Root CAUSE analysis

The INITIATING cause was the high speed of the truck.
Although this seems to suggest a human error (not respecting of the speed limit for the application), the event could be also classified as an operation cause: why the safety design had not considered this anomalous event and provided preventing measures?
