Description

A joint failure allowed a mixture of hydrogen and nitrogen into the atmosphere.
Failure of double cone joint on waste heat boiler in synthesis loop caused release of synthesis gas at 28.7 MPa and 170 degrees C after shutdown for maintenance.
There was no structural damage.
