Root CAUSE analysis

Environmental conditions (lightning) provided an unforeseen ignition source to the air-hydrogen flammable mixture already formed in the vent stack. Also contributing to a ROOT CAUSE is the lack of, or malfunctioning of a electrical grounding system able to neutralise the energy of the lightning.
