Fire on a CGH2 bottles trailer

Description

A trailer-tractor transporting hydrogen cylinders veered off the road to the right at around for unclear reasons and eventually tipped over to the left. Part of the 4,000 cubic meters of hydrogen caught fire.
The fire department extinguished the vehicle with foam.
The highway remained closed in both direction for the duration of the firefighting intervention and the following recovery of the vehicle.

Root CAUSE analysis

INITIATING CAUSE was a traffic accident. In absence of information on drivers and driver behaviours, the ROOT CAUSE can be attributed generically to a human error (wrong manoeuvre).
