Description

The bus drove into the garage and collided with a folded-out safety barrier. The upper corner of the roof was damaged. Also the first hydrogen tank was affected by the collision but without any hydrogen leakage

Root CAUSE analysis

The INITIATING CAUSE was a collision between a FCEV bus and a stationary barrier.
The safety barrier was not in the correct position. It was not folded in by the responsible technicians after the previous maintenance work.

The ROT CAUSE was a human error.
