Description

A truck transporting hydrogen and oxygen cylinders collided with a passenger vehicle at an intersection and overturned, causing all cylinders containers to fall onto the road. Gas leaked from four hydrogen containers and one oxygen container and ignited.
No measures to prevent the containers from falling over, such as ropes, had been taken.

Root CAUSE analysis

The INITIATING was a road collision (human error).
The ROOT CAUSE was failing to secure the trailer load with measures able to prevent falling also in accidental conditions.
